Aviora Healthcare is a Canadian home healthcare provider supporting seniors, adults and families with in home care across Ontario. The services include personal support, companionship, respite care, dementia support, meal preparation, light housekeeping, medication reminders, mobility assistance, overnight care, hospital to home support, palliative support, and nursing services where appropriate.

Their approach centres on continuity and on matching a caregiver thoughtfully to the person, with care plans built around their own routines, preferences and independence rather than a standard package. They put weight on clear communication with the family, consistent documentation and caregiver accountability.

The team works through the transitions where families usually need help most: hospital discharge, recovery at home, declining mobility, dementia, caregiver burnout, respite and palliative care. They coordinate with occupational therapists, insurers and discharge teams where a family authorizes it. Caregivers are screened for the role, which can include a Personal Support Worker certificate or equivalent, a Vulnerable Sector Check, CPR and First Aid, and references. Where regulated nursing is provided, the nurse holds current provincial registration.
